Nursing Home Transition and Diversion Housing Subsidy (NHTD)

The Nursing Home Transition and Diversion Housing Subsidy (NHTD) program provides alternatives to nursing home placement for seniors and disabled adults. The goal of the program is to provide low-income individuals with accessible, safe, affordable housing and at-home services appropriate to their needs. The dollar amount of subsidy assistance varies depending on the income of the family or individual and the approved rent for the unit. In order to qualify for the program, the resident’s rent and utilities must be below 110 percent of the U.S. Department of Housing and Urban Development (HUD) Fair Market rent. Recipients of the subsidy must be Medicaid-eligible and approved for the program by the New York State Department of Health.
